An issue has come up with PokerStars accounts related to yours which have been used as duplicates of others.
Your own account is temporarily frozen, and we need to verify your identity before we can even consider its reinstatement.
For this reason and as a security measure, please provide us with the following two documents:
1) A clear photocopy of your driver's license, front and back, or other government-issued photo ID
2) A clear photocopy of your last utility bill (telephone, gas, water or electric) indicating your name, address and statement date.
If by any chance you have no access to a utility bill under your name, a credit card statement or bank statement will suffice
Also please make sure to refrain from accessing PokerStars or allow others to access PokerStars from your computer as long as this investigation lasts as this could lead to a permanent block from the PokerStars websites.
This can be forwarded to us by Email as an attachment, or by fax to Costa Rica.
Please make sure that you write down your user ID when sending these documents via fax
